Morphine

Dosage; Titrate as per response, respiratory rate and sedation score.

IVinitial dose (for acute pain) for Adult:0.5-2.0mg at 3-5 interval.

SC/IM (Acute pain) for those of 20-39yrs: 7.5-12.5mg at 2hrs interval.

SC/IM (Acute pain) for those of 40-59yrs: 5-10mg at 2hrs interval.

SC/IM (Acute pain) for those of 60-69yrs: 2.5-7.5 at 2hrs interval.

SC/IM (Acute pain) for those of 70-80yrs: 2.5-5 at 2hrs interval.

SC/IM (Acute pain) for those than 85yrs: 2-3mg at 2hrs interval.

IV (Acute pain) for neonate: 0.01-0.02/kg at 1hr interval.

IV (Acute pain) for Infant: 0.05-0.1/kg at 4hrs interval. 0.01-0.04/kg at 1hr interval. Oral chronic cancer (for acute pain) for adult:2.5mg at 5hrs interval.

Drug Indication Prolonged relieve of severe and intractable pain acute or chronic; opioid adjunct during general anaesthesia; adjunct in acute pulmonary oedema; relief of severe dyspnoea.
Precautions Asthma, hypotension, hypothyroidism, pregnancy and breast feeding, prostatic hypertrophy, hepatic and renal impairment, children under 1year.
Contra-Indications Acute respiratory depression, risk of paralytic ileus, acute abdomen, acute alcoholic, hypersensitivity to codeine.
Side Effects Nausea and vomiting, dry mouth, anorexia, constipation, cramps, abdominal pain, lightheadedness, dizziness, sedation, sweating, facial flushing, arrhythmias, tachycardia, urinary retension or hesitancy, reduced libido, ureteral spasm, bronchospasm, muscle rigidity.

Special Information Its active metabolites have a long half-life hence it cannot be used over a long time in severe chronic illness. Its overdose is treated with naloxone for a number of days.
