Amikacin

Dosage Adult; IM/Slow IV 15 mg/kg daily in 2 divided doses,

increased to 22.5 mg/kg daily in 3 divided doses in severe infections;

max. 1.5 g daily for up to 10 days (max. cumulative dose 15 g).

Neonatal sepsis Extended interval dose regimen by slow intravenous injection over 3-5 minutes or by intravenous infusion.

Neonate, 15 mg/kg every 24 hours Multiple daily dose regimen by intramuscular or by slow intravenous injection or by infusion.

Neonate, Loading dose of 10 mg/kg then 7.5 mg/kg every 12 hours

Serious Gram-negative infections resistant to gentamicin

Mode Of Action

irreversibly binds to 30s sub-unit of bacterial ribosomes; blocks recognition step in protein synthesis and causes growth inhibition.

Drug Indication Severe infections in hospitalized patients due to susceptible gram-positive bacteria.
Precautions Neonates, infants and elderly (adjust dose and monitor renal, auditory and vestibular function); avoid prolonged use; conditions characterised by muscular weakness. Peak plasma concentrations of amikacin greater than 30 to 35mcg/mL or trough concentrations greater than 5 to 10mcg/mL should be avoided. Comparisons with equipotent doses found that amikacin affects auditory (cochlear) function to about the same extent as gentamicin.
Contra-Indications Pregnancy, lactation and myasthenia gravis
Side Effects Ototoxicity, nephrotoxicity, neuromuscular block and respiratory paralysis, neurotoxicity, vertigo, nausea and vomiting, blood dyscrasia, paraesthesia, skin rash, dry fever, tremor, hypotension, headache.
