Penicillin-V

Dosage Adult

Actinomycosis: Mild: 2-4 g/day in 4 divided doses for 8 weeks

Surgical: 2-4 g/day in 4 divided doses for 6-12 months (after I.V. penicillin G therapy of 4-6 weeks)

Erysipelas: 500 mg 4 times/day

Pharyngitis (streptococcal): 500 mg 3-4 times/day for 10 days

Prophylaxis of pneumococcal or recurrent rheumatic fever infections: 250 mg twice daily

Dosing: Pediatric

Pharyngitis (streptococcal): 250 mg 2-3 times/day for 10 days

Prophylaxis of pneumococcal infections:

Children <5 years: 125 mg twice daily

Children ≥5 years: 250 mg twice daily

Prophylaxis of recurrent rheumatic fever:

Children <5 years: 125 mg twice daily

Children ≥5 years: 250 mg twice daily

Mode Of Action	Inhibits synthesis of bacterial cell wall, causing cell death, hence bactericidal. required.
Drug Indication	Streptococcal pharyngitis, tonsillitis, otitis media, erysipelas, cellulitis.
Precautions	History of allergy, high dose in patients with impaired renal functions. Possible Jarisch-Herxheimer reactions when treating syphilis. Intrathecal administration not recommended.
Contra-Indications	Neonates of allergic mothers, penicillin and cephalosporin hypersensitivity.
Side Effects	GI disturbances, allergic reactions, anaphylactic shock, super-infections, sore mouth and tongue, black hairly tongue.
Dosage	500mg every 6 hours increased up to 1g every 6 hours in severe infections; child up to 1 year 62.5mg every 6 hours, increased up to 12.5mg/kg every 6 hours in severe infections; 1-6 years, 125mg every 6 hours, increased up to 12.5mg/kg every 6 hours in severe infections; 6-12 years, 250mg every 6 hours, increased up to 12.5mg/kg every 6 hours in severe infections.
Pregnancy Category	Category A1: Drugs which have been taken by a sizeable number of pregnant women and women of child-bearing age with no any established rise in the frequency of malformations or other direct or indirect detrimental effects on the foetus having been noted.
