Mode Of Action

The mechanism of action of amitriptylinne is not fully known but may be due to its increasing the synaptic concentration of norepinephrine and/or serotonin in the central nervous system (CNS). These neurotransmitters are thought to be increased through inhibition of their reuptake by the presynaptic neuronal membrane.

Drug Indication Depression disorders; obsessive-compulsive disorders; panic disorders, bulimia nervosa; cataplexy associated with narcolepsy; management of some types of chronic, severe, neurogenic pain.
Precautions Blood monitoring; history of epilepsy; pregnancy and lactation; history of mania; close angle glaucoma; concurrent electroconvulsive therapy; anaesthesia; elderly; hepatic impairment; psychosis; withdraw gradually.
Contra-Indications Severe hepatic disorders; acute recovery phase following myocardial infarction; hypersensitivity to tricyclic drugs; MAOIs.
Side Effects Sexual function impairment, seizures, and nausea and vomiting may occur more frequently with it than with other tricyclic antidepressants.

Treatment of OCD: Oral: Initial: 25 mg/day; may gradually increase as tolerated over the first 2 weeks to 100 mg/day in divided doses; Maintenance: May further increase to recommended maximum of 250 mg/day; may give as a single daily dose at bedtime once tolerated

Note: The safety and efficacy of clomipramine in pediatric patients <10 years of age have not been established and, therefore, dosing recommendations cannot be made.
