Remifentanil

Dosage ; 0.5-1mcg/kg/min as IV inf.

Maintenance: 0.5-1mcg/kg/min as IV bolus given 2-5 minutes

or 0.05-2 mcg/kg/min given at an interval of 2-5 minutes.

Spontaneous ventilation: 0.04mcg/kg/min IV infusion initially

then stabilized at 0.025-0.1 mcg/kg/min.

Mode Of Action

opioid receptor agonist

Drug Indication Induction and/or maintenance of general anesthesia during surgical procedures; continuation of analgesia into the immediate post-operation period; during transition to longer-acting analgesia; sedation in mechanically ventilated intensive care patients.
Precautions It can cause muscle rigidity, particularly of the chest wall or jaw; this can be managed by the use of neuromuscular blocking drugs.
Contra-Indications Epidural and intrathecal use.
Side Effects Post-operative shivering, nausea and vomiting, hypotension, apnoea, skeletal muscle rigidity, bradycardia, acute respiratory depression, post-operative hypertension, pruritus.
