Mode Of Action

Selegiline is an irreversible inhibitor of monoamine oxidase type B (MAO B), which is involved in the oxidative deamination of dopamine in the brain. It is also thought to interfere with dopamine re-uptake at the synapse.

Drug Indication Parkinsonism, (but not drug-induced extrapyramidal symptoms). Can be used alone or with levodopa.
Precautions Elderly, angina, arrhythmias, psychosis, pregnancy, breast-feeding, impairment, poorly controlled hypertension, avoid in active ulceration, need to reduce the dose of L-dopa by 10-50% when concurrently administered.
Side Effects GI disturbances, psychosis, sore throat, hypotension, skin reactions, vertigo, tremor, agitation, back pain, depression, sleep disturbance, muscle cramps, difficulty in passing urine.

Capsule/tablet: 5 mg twice daily with breakfast and lunch

Administration

Oral: Orally disintegrating tablet (ZelaparŪ): Take in morning before breakfast; place on top of tongue and allow to dissolve. Avoid food or liquid 5 minutes before and after administration.

Topical: Transdermal (EmsamŪ): Apply to clean, dry, intact skin to the upper torso (below the neck and above the waist), upper thigh, or outer surface of the upper arm. Avoid exposure of application site to external heat source, which may increase the amount of drug absorbed. Apply at the same time each day and rotate application sites. Wash hands with soap and water after handling. Avoid touching the sticky side of the patch.

Use

Adjunct in the management of parkinsonian patients in which levodopa/carbidopa therapy is deteriorating (oral products); treatment of major depressive disorder (transdermal product)
